It’s Time.

Discussing your advance health care choices and end-of-life care with your loved ones is one of the most difficult conversations you will ever have—it’s also one of the most important to ensure your wishes are known before a crisis occurs. All adults are welcome to our in-person clinics on how to Begin the Conversation about advance care wishes. There is no cost to attend.

Advance care planning is a gift you give your family. Our knowledgeable and understanding staff will help you communicate your wishes now, before a crisis occurs, to your family and doctors.

Begin the Conversation about your advance care wishes

Our Begin the Conversation program
encourages people to make, document and communicate healthcare choices and evaluate what is important to them. We also help guide you through discussing these wishes with loved ones, doctors, and putting these plans in writing.

We believe it’s time to face “the elephant in the room” and talk about your advance healthcare and end-of-life wishes. Your loved ones will appreciate your consideration and preparedness. Advanced care planning requires  completing documentation such as:

  • A Healthcare Power of Attorney
  • A Living Will
  • A MOST form (Medical Order for Scope of Treatment) or a POLST form (Physician’s Orders for Life-Sustaining Treatment)

We will help you tackle this difficult subject

We have an extensive array of tips and tools to help you communicate your wishes to those you love. We can also explain and help you complete the forms you need to document your plan. It’s vital to complete planning now before an emergency arises.  It’s also important to ask those you love if they have considered their final wishes as well.

Making these decisions now will be a relief and comfort for your loved ones if you are seriously injured, become suddenly ill, or develop a life-limiting illness that makes it unable for you to communicate your wishes.

Our Begin the Conversation program partnered with New Hanover Regional Medical Center (NHRMC) to assist their employees in getting their advance care plans in place, and help them learn how to talk to patients about documenting theirs as well.

Only 10% of our patients take advantage of the full Medicare hospice benefit which includes ALL medical care, medications, equipment and visits related to diagnosis.
