Join us at one of our monthly Begin the Conversation Advance Care Planning workshops. Learn why an advance care plan is important, how to discuss your wishes with loved ones and doctors, and what documentation you need to get your plan in place.
This workshop provides participants with our free advance care planning workbook, legal documents needed to get a plan in place, and the opportunity to learn more about ensuring your healthcare wishes are known. There is no cost to attend.
