Comprehensive Support for Caregivers and Their Loved Ones
Our services are designed to meet families where they are, providing guidance, relief, and support through every stage of memory loss.
Our program includes:
- Care Consultations to assess needs and guide care planning.
- Education and Training, including SAVVY Caregiver classes and peer support groups.
- Respite Care from trained dementia volunteers to give caregivers time to rest.
- Therapeutic Programs like Music & Memory and expressive arts.
- Advanced Care Support with smooth transitions to home care, Palliative Medicine, and Hospice Care when needed.
CMS GUIDE Program
At LifeCare Memory Partners, we are proud to be part of the CMS GUIDE Model (Centers for Medicare & Medicaid Services – Guiding an Improved Dementia Experience), a groundbreaking initiative launched in 2024 to support Medicare beneficiaries living with dementia and their caregivers.
As the only local provider in our region (east of Raleigh, south of Virginia, and north of Charleston), we offer comprehensive dementia care services designed to improve quality of life, provide 24/7 support, and ease the burden on caregivers.
