At LifeCare Memory Partners, we help caregivers find balance and strength through education, emotional support, and compassionate care. Our program provides tools, training, and resources to improve quality of life for both caregivers and their loved ones, while connecting families with broader LifeCare services, including palliative medicine and hospice care when needed.

Understanding the Challenge

You Don’t Have to Navigate Dementia Care Alone

Every 65 seconds in the U.S., someone is diagnosed with dementia. Most caregiving falls on family members who may not have the training or resources to manage its demands. LifeCare Memory Partners offers programs designed to ease the journey, reduce caregiver stress, and strengthen connections between families and their loved ones.

What LifeCare Memory Partners Provides

Comprehensive Support for Caregivers and Their Loved Ones

Our services are designed to meet families where they are, providing guidance, relief, and support through every stage of memory loss.

Our program includes:

  • Care Consultations to assess needs and guide care planning.
  • Education and Training, including SAVVY Caregiver classes and peer support groups.
  • Respite Care from trained dementia volunteers to give caregivers time to rest.
  • Therapeutic Programs like Music & Memory and expressive arts.
  • Advanced Care Support with smooth transitions to home care, Palliative Medicine, and Hospice Care when needed.

CMS GUIDE Program

At LifeCare Memory Partners, we are proud to be part of the CMS GUIDE Model (Centers for Medicare & Medicaid Services – Guiding an Improved Dementia Experience), a groundbreaking initiative launched in 2024 to support Medicare beneficiaries living with dementia and their caregivers. 

As the only local provider in our region (east of Raleigh, south of Virginia, and north of Charleston), we offer comprehensive dementia care services designed to improve quality of life, provide 24/7 support, and ease the burden on caregivers.

Building a Dementia-Friendly Community

Changing How Our Communities Understand Dementia

LifeCare Memory Partners also leads community education and training sessions to raise awareness and foster compassion for those living with dementia. By helping individuals and organizations learn how to communicate effectively and respond with understanding, we’re creating a more dementia-friendly community—one conversation at a time.

Our dementia care connects seamlessly with other LifeCare programs, including Palliative Medicine for symptom management and Hospice Care for advanced support.
