On January 1, 2020, Lower Cape Fear Hospice formally changed its name to Lower Cape Fear LifeCare to better represent the spectrum of care and support they provide those in our community.
What would happen if you suddenly fell ill or were in an accident and were unable to communicate your healthcare wishes? An advance care plan ensures your wishes are known and relieves the burden of guessing for loved ones at an already stressful time.
Join us at one of our monthly Begin the Conversation Advance Care Planning workshops. Learn why an advance care plan is important, how to discuss your wishes with loved ones and doctors, and what documentation you need to get your plan in place.
This workshop provides participants with our free advance care planning workbook, legal documents needed to get a plan in place, and the opportunity to learn more about ensuring your healthcare wishes are known. There is no cost to attend.
